Death certificates may be obtained from any county in Minnesota for deaths occurring after 1997. Deaths occurring before that time will need to be purchased at the county where the death occurred or county of residence.

A Death Certificate Application form is needed to request a Death Certificate. Your signature must be notarized if applying by mail.
