ALL APPLICANTS MUST APPEAR IN PERSON

We are processing new passports by APPOINTMENT ONLY.  Our hours for appointments are between 8:00 a.m. - 12:00 p.m. or between 1:00 p.m. - 4:30 pm.  Please call 507-238-3213 to schedule your appointment.

Passport Information

Passport Required For:
  • All air travel to or from Canada, Mexico, Central and South America, the Caribbean, and Bermuda.
  • Check requirements for sea travel and land border crossings at the U.S. Department of State's web site: https://travel.state.gov/content/travel.html

To obtain a United States Passport you will need to present the following:

1. Completed passport application form (you MUST sign in person) - obtain from the County Recorder's Office or the Passport Agency's web site https://travel.state.gov/content/travel.html and click on "Application for Passport".

2. A certified copy of birth certificate from the county or state level (not a hospital certificate) and photocopy of the same.

3. One passport photograph that meets passport requirements.

4. Valid driver's license for identification. Additional identification required for out of state drivers license.

Minors under 16 must appear with both parents and parents must present valid driver's licenses. Minors 16 and older, if they have a valid driver's license, may appear without a parent, but should have a parent's check to show consent. If a minor 16 years old or older has no ID, one parent must appear with the minor.

Under 16:
$100.00 check to US Department of State
$35.00 cash, credit card or check to Martin County Recorder

Passport Card  (Does NOT cover international air travel)
Adult: (16 & over)
$30.00 check to US Department of State
$35.00 cash, credit card or check to Martin County Recorder
Under 16:
$15.00 check to US Department of State
$35.00 cash, credit card or check to Martin County Recorder

Passport Book & Card at the same time
Adult: (16 & over)
$160.00 check to US Department of State
$35.00 cash, credit card or check to Martin County Recorder
Under 16:
$115.00 check to US Department of State
$35.00 cash, credit card or check to Martin County Recorder

6. If one parent cannot appear with the child to sign, you must have a signed and notarized statement from that parent. A form can be found under the Passport Agency's web site: https://travel.state.gov.
